We want to make sure you can afford your procedure. We can help you connect with funding organizations that will help you if you qualify. Payment is due on the day of your procedure. Payment can be made by cash (personal checks are not accepted), credit card, or insurance. Our patient advocates are available to talk with you about your individual payment needs.
The following credit cards are accepted: VISA, MasterCard and Discover. If the cardholder is unable to be present on the date of service, you must complete a Remote Credit Card Authorization Form for the clinic where services will be performed (located directly below). The cardholder must complete this form with a copy of their identification and both sides of the credit card. Patients must bring the completed form with the required information to their appointment.
Our services are covered by most insurance plans. Please have your insurance card with you when you call to schedule your appointment as coverage needs to be verified prior to your visit.
Services in Connecticut and New Jersey are covered by CT and NJ Medicaid. Please have your Medicaid card with you when you call to schedule your appointment as coverage needs to be verified prior to your visit.
